I consent to and authorize CPFI to utilize, process and retain my personal information in accordance with the applicable data privacy laws, privacy policies, administrative orders, and government issuances for the purpose of completing the registration for the Supplier Payment Portal of CPFI and to enable procurement to advise suppliers during collection. I hereby affirm my right to be informed, object to the processing, access and rectify, suspend or withdraw my personal data and be indemnified in case of damages pursuant to the provisions of the Data Privacy Act of 2012 and its corresponding implementing rules and regulations and other government issuances.
